PENALTY CUP REGISTRATION FORM

Please submit one Registration Form per team of 5 Players and 2 Goalkeepers plus one Coach. A team can be boys' or girls', or a mixed boys and girls team.

Get ready with details of the participants (full name, NRIC Mykad number or passport number) and their parent (name and email) before attempting to fill-up the Registration Form. Thank you!

CLOSING DATE for submission of registration is 1st January 2025.

PARTICIPATION FEE
RM550 per team.
RM490 Early Bird Price for payment by 25th December 2024.
